Individual Written Report on a Technological Controversy
Report Template & Instructions For Use
The template on the following pages is an example of an acceptable format for a report.
Prepare your Draft Plan by completing the template, inserting relevant headings, content (including evidence and in-text citations) and a reference list. Your completed Report Plan should be at least 3 pages long, while the Report will be longer. Add your name & ID to every page, and submit your completed work by the due date, attaching a Cover Sheet to the front of the hard copy. Also submit a soft copy via Turnitin. Time will be available to do this in your class on the due date.
Most of the information in the template needs to be in dot point form only. However, if you submit all the required text so that your Draft Plan is a full draft of your Report, you will receive more feedback,which should help you edit and improve your final Report.

Introduction: Dot points only required. Reference/(s) can be cited in the background statement. Cite them in the short form, eg Chang (2007, p. 219) states that ………..…. or ………………. (Chang 2007, p. 219). Note that page number(s) is(are) required in citations where applicable. The full publication details must be written in the Reference List.
Definitions: Dot points only required.
Method(s): Dot points only required.
Results & Discussion: Dot points only required for contents of each sub-section. Reference/s should be cited with every statement of fact, and there should be at least one citation in each sub-section.
Conclusion: Dot points only required.
Recommendations: To be written in full.
Reference List: To be written in full. All references to be formatted according to the UTS Harvard guide, eg in alphabetical order according to family name; no dot points or numbering should be used at the beginning of each reference.

List each section and the page number of the beginning of each section.
Write this last.
Executive Summary
Start this section on a new page. Do not number the title.
Write this after you have written the report.
Use alternative numbering for this page, as it is not part of the main body of your report.
Write a brief summary of the contents of your report (about 4 to 6 sentences in one paragraph).
Include at the end of this section a copy of your recommendations that are listed in Section 6.
Address your report to the management head of the organisation that you are reporting to, using their title only (do not use an individual’s name). To do this you may have to research an organisation’s structure.
1. INTRODUCTION 引言

Start numbering section titles with this section.
Write the Introduction as one paragraph only.
Give a brief background to the issue. One to three sentences only. References can be cited.
Then write a statement of purpose about the controversy that has been researched and is to be reported on. No references needed.
Finally, list the topics covered in the different sections of the report, in their order. Use sequencing/ linking words. No references needed.

This section should contain at least 3 sub-sections, that discuss:
• The current ethical controversy
• A history of the ethical controversy.
• Possible strategies for developing the technology ethically in the future. This sub-section should contain all the information you will need to write the 3 recommendations.

Discuss the actors (objects, individuals and institutions, such as technologies and stakeholders such as innovators, researchers, business corporations, consumers, regulators, governments, politicians, media, legal institutions, ethics organisations, activists) and contextual factors (eg, social, cultural, economic, environmental, religious, global, national, local) and their relations in the development of the technology & ethical controversy up to the present & in the future.#p#分頁標題#e#
Write a topic sentence and concluding sentence at the beginning and end of this section. In addition, for each sub-section, write:

• At least 3 sentences in the body of your analysis

4. CONCLUSION
Do not start a new page.
Restate the purpose of the report and sum up the findings of your research.
Do not add any new information.
Do not use referencing, as there should be no new information in this section.
Write 3 - 5 sentences. Include a topic and concluding sentence.
5. RECOMMENDATIONS 建議
Provide 3 recommendations, using information provided in Sub-section 3.3. Use numbered recommendations

Each recommendation should identify the specific action(s) to be taken and a deadline for when the action should be taken.
No referencing needed, as there should be no new information.

Use the Harvard referencing system. List a minimum of 3 references:
• 1 reference from the FTS101 Reader
• 1 reference from any FTS101 resource
• 1 reliable and relevant reference you have sourced yourself.
Use single spacing within each text and double spacing between texts. Justify the page to the left. Use a hanging indent for the second and later lines.
Do not use dot points or numbering for each text listed.
Do not include unreliable references, such as a Wikipedia site or answers.com.
